Performs an initial and ongoing assessment to determine the patient''s cludes data from the patient/significant other(s) in the assessment as appropriate.Prioritizes data collection according to patient''s immediate needs. Develops/reviews/revises and individualizes the plan of care to address the patient''s nursing care needs for optimum patient care outcomes. Provides education and support for the breast feeding mother and baby during inpatient collaboration with Lactation Consultants, assesses inpatients with an identified lactation risk factor.Consults with staff nurses regarding patient assessment/risk factor.Documents patient assessments and interventions. Identifies potential problems in breast feeding mother and baby during the inpatient or outpatient collaboration with Lactation Consultants, assesses inpatients with an identified lactation risk factor.Consults with staff nurses regarding patient assessment/risk factor(s).Collaborates with inpatient Lactation Consultants to support initiation of breast milk production and supply.Documents patient assessments and interventions. Provides lactation instruction and structs mothers (adolescents and adults) on all tasks necessary to initiate and maintain lactation during the infant''s hospitalization; may include prenatal instruction and post-discharge follow-up.Evaluates breastfeeding oral motor mechanics of infants and milk transfer, making appropriate recommendations and management municates plan to health care team members caring for the infant, E.g., staff RN, NNP, MD.Documents observations, assessments and recommendations made during consultation in patient record.Assure follow-up per phone. Instructs in lactation maintenance and human milk storage.Performs all tasks necessary to ensure the mother''s adherence to departmental policies related to milk collection, storage and transport.Obtains information on maternal medication use and evaluates its compatibility or prohibitive status related to breastfeeding or expressed milk feeding. Provide ongoing professional education on breastfeeding issues.Prepares formal inservices for staff and/or community.Utilizes informal opportunities to teach staff by hands-on and examples.Participates in orientation of new employees.Serves as a resource for student experiences.Develops and implements training related to breastfeeding instruction and lactation support to community affiliated sites'' nursing/medical staff (outreach). Performs a telephone assessment of discharged breast feeding mothers and babies.Utilizes appropriate telephone communication techniques for accurate itiates referral as outpatient or to provider as indicated.Documents phone assessments.Essential Functions II Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management. Follows and understands the mission, vision, core values, Employee Standards of Behavior and company policies/procedures. Other duties as assigned.
