It''s a team of extraordinary medical professionals with the latest ideas for keeping patients healthier.Esse Health is currently seeking a full-time Care Manager - RN to cover Southwestern Illinois.The RN Care Manager is a member of the care management team and works with the Primary Care Physician to improve the health of patients. The RN will use the principles of nursing assessment, diagnosis, planning, implementation, and evaluation to deliver care. The Care Manager will focus on care coordination and discharge planning for Esse patients in several hospitals and skilled nursing facilities in the Illinois region. Essential to the role of a care manager is the ability to collaborate and participate in care coordination activities that meet the patient needs. These skills are essential to reducing barriers when managing discharge planning, transitions of care, and assisting patients throughout their health care journey.The RN Care Manager will incorporate both face-to-face and telephonic based services utilizing care management strategies. The RN is responsible for working collaboratively with the patient, family, physicians and the entire health care team.
