Job DescriptionWe are looking for an experienced Clinical Liaison RN to join our Valley''s Best Hospice / Pacific Family Hospice team in Anaheim .The Clinical Liaison serves as a member of the Hospice Interdisciplinary Team with a principle role ofdeveloping a hospice presence in assigned facilities. The Clinical Liaison will serve as a consultant onhospice/palliative care related issues, provide clinical reviews and expertise on hospice appropriatenessand build professional relationships for improved patient access to end-of-life care.PRIMARY JOB DUTIES AND RESPONSIBILITIES:For the purpose of improving care for the terminally ill, develop a Valley''s Best Hospice presence in assigned medical group or otherfacilities, attending rounds and care conferences when possible, conferring with providers, speaking at meetings and participating inother activities as available.JOB SUMMARY:The Clinical Liaison serves as a member of the Hospice Interdisciplinary Team with a principle role ofdeveloping a hospice presence in assigned facilities. The Clinical Liaison will serve as a consultant onhospice/palliative care related issues, provide clinical reviews and expertise on hospice appropriatenessand build professional relationships for improved patient access to end-of-life care.PRIMARY JOB DUTIES AND RESPONSIBILITIES:Evaluate patient appropriateness for hospice or palliative care through report from other professionals, review of medical records oronsite meeting with patient/family.Confer with assigned medical group staff and patient''s physician.Coordinate hospice admissions, as appropriate, based on completed evaluations and patient wishes.Work with Valley''s Best Hospice clinical and admissions teams to ensure timely transportation, admission, DME delivery and medication.Develop and maintain contact with key medical groups to provide ongoing updates on Valley''s Best Hospice Services.Provide exemplary customer service to physicians, discharge planning staff, case managements staff, facility personnel, patients and patient families at all times.Abide by all appropriate guidelines established by all assigned medical groups or facilities as appropriate.Maintain working knowledge of community resources and assists assigned medical group or facility in accessing those resources which may not be directly offered by Valley''s Best Hospice.Serve as a liaison with group or facility on behalf of Valley''s Best Hospice to coordinate care for patients admitted to hospital orseeking care outside of Medicare Hospice Benefit.Requirements:Current valid California Registered Nurses license.Ability to develop strong working relationships with physicians, specialists, case managers, managed care organization staff, hospitalstaff and nursing facility personnel. Ability to effectively communicate effectively to patients and families.BLS requiredMinimum of one year experience as a professional nurse. Home Health or Hospice experience a plus.Current Driver''s License and proof of insurance.
