Optum is looking for a Full-Time P alliative Care Nurse Practitioner to join our team in the South Bay area . The clinicians we seek are those who practice medicine with a focus on patient care, not volume. Our initial online application process will take you less than two minutes to complete!Position Details Clinician to join our newly redesigned Palliative Care ProgramMonday - Friday : 8am - 5pmAverage Patients Per Day ~10-12 PPD | 60 mins for new patients and 30 min follow up appointmentsPatient Mix : >65 with chronic medical conditions eg. CHF, COPD, DiabetesNo weekend or after hours clinics but phone call coverage required in rotation with other cliniciansShard call with all high risk providers, one weekend every few months but do not need to be home boundA majority of the time will be spent between an office setting and visiting patients in a home settingDedicated support staff : Care Partner or Social Worker assigned to each patient depending on needsSupport in Clinic : MA, LVN, and Pharmacist to clinic panel size of ~150 patientsRequirements Unrestricted CA Medical License as a Nurse Practitioner3+ years of previous and current experienceCurrent CA DEA certificate required prior to start dateCA Drivers license active in good standingBoard Certified as a Nurse PractitionerFull COVID-19 vaccination is an essential requirement of this role.
