Do you have the Hospice Heart?Your Life as a Nurse PractitionerYour role is very important to our patients and organization. We meet our patients where they are and this position will serve our patients in allcounties. We will rely on you to ensure that all patients you visit have addressed any nursing needs or determine the need for other team members to assist in providing resources. To achieve this you will assist the patient or family in determining the best plan of care that meets their needs. We are a team and it is your role to direct other team members and ensure that they are exceeding our patient''s expectations. We want to share your extraordinary care and need to make sure that we are able to process billing. You are responsible for documented your care and ensuring your team members are as well. This documentation should paint the picture of your patient, so if another team member is given the honor of serving that patient they are aware of their expectation and know what is currently working. Your focus will always be on what is your patient''s greatest need and providing ''Care, Hope, and Support for all when Illness threatens life.'' This position provides is designed to have flexibilty in continuing care while other team members are out or supplement high levels of admissions. As an Expansion Nurse Practitioner, your goal should be that your patients experience is their loved ones memory.Job QualificationsGraduate of approved school of professional nursingRecipient of a current license by Texas Board of Nurse Examiners as and Advanced Clinical Nurse Specialist or Practicing Nurse Practitioner.Recipient of a current licenseunder Texas Medical Practices Actasan Advanced Clinical Nurse Specialist or Practicing Nurse Practitioner.Ability to understand the physical, psychosocial and spiritual concerns/and problems of terminally ill patients in a home setting.Possess critical thinking passionateEffective Communication both written and orally.For more information, or to apply now, you must go to the website below.
