Job DescriptionRemote role with travel required up to 50% of the time to support the business and drive improvements and processes. Preference to reside near Chicago, IL. Schedule: Monday-Friday standard business hours with flexibility needed to meet the needs of the client. This role is key to facilitating transformation of the care that is delivered to ACO REACH attributed lives, strengthening the patient-centric collaboration between CVS ACO Inc. and our provider networks. The role is provider/clinic facing and is responsible for driving to continuous improvements in quality and patient experience, decreasing the total cost of care and translating to profitable growth. Analyze and report on provider quantitative performance and operational capabilities and to execute a strategic roadmap. This role will have in depth knowledge of each of the practices assigned and consistently collaborate with all stakeholders. The role will also be responsible for supporting clinical performance meetings where applicable.Pay RangeThe typical pay range for this role is:Minimum: 82,000Maximum: 176,800Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications-2+ years of prior work experience managing operational KPIs in a healthcare setting -2+ years of managed care experience, preferably Medicare experience - One of the following is required: RN (Registered Nurse) or LCSW (LICENSED CLINICAL SOCIAL WORKER)Preferred Qualifications Previous Medicare experience Experience working in an ACO (Accountable Care Organization) setting Demonstrated analytic and problem-solving skills Ability to work independently (this position will be work from home) Excellent written and verbal communication and presentation skills Demonstrated interpersonal and delegation skills required Ability to communicate effectively with providers, staff, and other leaders both in oral and written formats. Ability to evaluate and interpret data and identify areas of improvement.
