Job DescriptionMonday-Friday 8am-5pm EST. This is a full-time remote worker opportunity in Ohio. Qualified candidates must reside in Ohio and within 50 mile radius of Columbus city center.Business Overview:As part of the bold vision to deliver the "Next Generation" of managed care in Ohio Medicaid, OhioRISE (Resilience through Integrated Systems and Excellence) will help struggling children and their families by focusing on the individual with strong coordination and partnership among CMEs (Care Management Entities), MCOs (Managed Care Organizations), vendors, providers, PCSAs (Public Children Services Agencies) and the Ohio Department of Medicaid to support specialization in addressing critical needs. The OhioRISE program is designed to provide comprehensive and highly coordinated behavioral health services for children with serious/complex behavioral health needs involved in, or at risk for involved in multiple child-serving systems.Position Summary:This is a full time remote worker opportunity in Ohio. Qualified candidates must reside in Ohio and within 50 mile radius of Columbus city center. Discharge Coordinators are vital members of the OhioRISE Utilization Management (UM) Team. This role serves liaison functions connecting UM with multiple aspects of care coordination. For embedded discharge coordinators, this extends further to integrating into the day-to-day functioning of the assigned hospital.Fundamental Components: Assist hospital staff, care coordinators and Child & Family Teams in curating appropriate and individualized discharge plans for all OhioRISE members' post-hospital care. Discharge plans are to be comprehensive, tailored and feasible in order to reduce the likelihood that members receive duplicative services or are readmitted to the hospital. Utilize negotiation skills to secure appropriate service options to match the member's healthcare needs. Provide coaching, education and technical assistance to the member's care coordinator and Child and Family Team around the member's benefits and contracted services. Interface with various OhioRISE departments including Utilization Management (UM), Care Coordination (CC), Transition of Care (TOC) and Care Management Entities (CME) around securing the recommended services and supports for members to discharge successfully from the inpatient hospital. Be responsible for entering required information into the Ohio CANS IT System which helps determine and confirm member eligibility for OhioRISE. Render technical assistance to hospital staff regarding OhioRISE UM processes for services on the discharge plan that require prior authorization. OhioRISE Discharge Coordinators will be fully dedicated to their assigned hospital(s) but may be required to provide cross coverage to other hospitals on an as-needed basis. Embedded OhioRISE Discharge Coordinators will be physically located at their assigned primary hospital site.Pay RangeThe typical pay range for this role is:Minimum: 19.50Maximum: 40.10Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications Must reside in Ohio and within 50 mile radius of Columbus city center. Minimum of 2 years of experience in Behavioral Health, Social Services, or related field working with multi-system involved youth. 2+ years' experience with personal computer and keyboard navigation is required for internal/external computer systems and MS Office Suite applications including Word and Excel. Must have reliable transportation and the ability to travel in-state up to 50% of the time. Travel will be to hospitals in the assigned geographical area.Preferred Qualifications Medicaid and or managed care experience is a plus.Education Minimum of a bachelor's degree in a Behavioral Health or Human Services field (e.g., Social Work, Psychology, Counseling, Marriage and Family Therapy).
