Job Description We are offering a $5,000 new hire bonus for this positionThe LPN Clinical Liaison will be aligned to hospitals in the Medford OR area. As an LPN Clinical Liaison with Coram CVS/specialty infusion services, you will develop and maintain relationships with physicians, social workers, discharge planners and other referral sources, gathering all pertinent discharge information and working collaboratively with patients and clinical staff providing coordination of care to ensure a smooth transition from a medical facility back into the home. Must have excellent written and verbal customer service skills and advanced computer skills in order to interact with key stakeholders and patients.Our LPN Clinical Liaisons ...- Maintain and growing core therapy admits through customer relationships and providing responsive customer service with assigned accounts.- Evaluates current and future clinical and service needs for existing and new accounts.- Collaborates with prescribers, case managers, payers, referral sources, home health agencies, and Company staff to facilitate patient discharge and coordination of care.- Performs nursing care in states where an LPN is able to perform nursing care with infusion therapy, at the direction of a company Registered Nurse. (Activities may be limited. Both the LPN and RN must comply with state regulations for the scope of practice of an LPN).- As allowed by state regulations, provides patient education regarding therapy and supplies and/or connects patient to an infusion pump in order to facilitate discharge from a facility.- Completes all therapy-specific clinical modules and pump competencies prior to teaching, administering, and/or connecting infusion therapies to a patient.- Demonstrates thorough knowledge of core therapy referrals, qualifying criteria both by payor source and branch.- Conducts in-services and continuing education programs for referral partners and/or their support staff.- Introduces new services, therapies, drug protocols, and products to referral sources.- Recognizes opportunities for expanding the business relationships with existing accounts and involves the Territory Manager or Regional Sales Manager at the appropriate time to present and negotiate Coram's other programs.- Communicates/coordinates discharge planning activities for new patient referrals with branch personnel (admissions, clinical, operations, reimbursement) and referral partners.- Assists with activities to achieve favorable branch net revenue and profit/loss that meet Company objectives.- Participates in all scheduled sales conference calls, nursing, and branch meetings as directed.- Performs other duties and responsibilities as assigned.Pay RangeThe typical pay range for this role is:Minimum: 38,700Maximum: 79,500Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls.
