Job DescriptionThis is work from home flexible position with upto 10% travel to the Chicagoland area. Candidates can be located anywhere in IL or neighbouring states within commutable distance of Chicago land area.Behavioral Health Clinician responsible for the care coordination and care management needs for the Medicaid population in Southern Illinois. Utilizes advanced clinical judgement and critical thinking skills to facilitate appropriate behavioral and physical health needs, linkage with providers and services, provides an explanation of benefits and resources available to the member. Addresses escalated concerns as it relates to access to care, gaps in providers and services, and timeliness of care. Collaborates with the member's care team to effectively coordinate services. Develops a person centered care plan with the member based of a comprehensive assessment inclusive of social determinants of health. Incorporates motivational interviewing to ensure maximum member engagement. Monitoring, Evaluation. and Documentation of Care in collaboration with the member and their care team. Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policy and procedurePay RangeThe typical pay range for this role is:Minimum: 28.25Maximum: 60.50Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications-Minimum of 3 years in direct patient care in a behavioral health setting required- Active and unrestricted BH license (LCSW, LCPC, LMFT) in the state of IL.-Willing and able to travel up to 10% of their time to chicagoland area.Preferred Qualifications-2+ years of Case Management experience.-Team oriented is strongly preferred with experience working in multiple teams.CCM CertificationCADC (Certified Alcohol and Drug Counselor)Managed Care Organization experience working with the Medicaid population preferredCrisis Intervention skills, CPI certificationEducationMasters Degree in a social science such as Social Work, Counseling, PsychologyBusiness OverviewBring your heart to CVS Health Every one of us at CVS Health shares a single, clear purpose: Bringing our heart to every moment of your health.
