We are seeking a compassionate, dedicated, and licensedPer Diem Physical Therapistto provide high-quality rehabilitative care as part of our Visiting Nurse Association (VNA) team. About the Role: As aPer Diem Physical Therapist , you will deliver expert, patient-centered care to individuals in their homes, helping them regain strength, mobility, and independence. You'll work closely with physicians, caregivers, and a supportive clinical team to develop personalized care plans and achieve meaningful outcomes. Key Responsibilities:Provide in-home physical therapy services as prescribed by the attending physician. Conduct assessments and create individualized care plans to restore function and relieve pain. Educate patients and families on therapy goals, assistive devices, and safety practices. Collaborate in case conferences and discharge planning. Maintain detailed, timely documentation of visits and progress notes. Supervise Physical Therapy Assistants as necessary. Comply with COVID-19 policies, including symptom monitoring and PPE guidelines. Qualifications:Graduate of an accredited Physical Therapy program. Licensed Physical Therapist in the state. At least 1 year of experience in an institutional or home care setting preferred. Valid driver's license, reliable vehicle, and proof of insurance. Strong interpersonal skills and a passion for home health care. Physical Requirements:Ability to lift up to 50 lbs and assist patients weighing over 100 lbs. Ability to travel to client homes and operate portable therapy equipment. Frequent standing, bending, and moving throughout the workday.
