The LPN will coordinate care, administer medication, provide education to youth and families, and ensure compliance with medical standards.Key Responsibilities: Assess health needs of youth through home visits and direct contact Coordinate care with physicians and service providers Administer medications and monitor responses Educate youth, families, and caretakers on health conditions and self-care Maintain accurate medical records and ensure regulatory compliance Participate in team meetings and care planning Foster a trauma-informed, supportive environmentQualifications: Graduate of accredited LPN program NYS LPN license & current CPR certification Strong communication and teamwork skills Experience with trauma-informed care preferred Must be able to travel with youth to appointments
