Job Description Summary The South Shore Visiting Nurse Association (SSVNA) Community Liaison is responsible for the referral management of patients.Primarily located at South Shore Hospital, but will cover community providers as needed. Job Description Essential Job Functions Identifies and prioritizes patients at community health care facilities who have requested home care services from SSVNA Acts as consultant to SSH care managers/SSH staff regarding home care programs, community resources and alternative care settings Establishes and develops relationships with colleagues, physicians, referral sources, patients and family members to facilitate the referral to admission process and acts as liaison between all parties Explains the types of care and services the SSVNA offers to patients and their families Recruits' referrals and coordinates with the Central Intake department in assisting the coordination of admissions while aligning with SSVNA’s daily capacity Assesses incoming patient’s physical, social and psychosocial needs to ensure they meet the agency’s admission criteria. Consults with referral sources on complex patient discharges and coordinates patient’s care with SSVNA Obtains financial information, as necessary Establishes open lines of communication with referral sources to promote good community relations Works to provide excellence in service to existing accounts and strives to increase referrals Participates in appropriate professional, community organizations, events and outreach activities Reviews weekly Admission and Referral Reports and Dashboards.
