Whether we’re working on the frontlines in Programs or working behind the scenes in Support Services, we feel privileged to use our education, experience, and gifts to love people and bring glory to God.The Lasting Solutions Specialist serves as a critical link between individuals and CityTeam’s Elevate Level programs, communities, and the services and knowledge they need to thrive in the assigned area.This dual-role position combines the resource-connection expertise of a Community Navigator with the capacity-building functions of an Educator. The role aims to both support individuals in accessing services and empower them through targeted education and outreach that promotes self-advocacy, various types of literacy (language, financial, technology, etc), and community participation.The Lasting Solutions Specialist is a part time non-exempt position. All of this is wrapped up in a Christian environment that values Teamwork, Accountability, Humility, Innovation, and Joy.Key Responsibilities:Research, maintain, and disseminate a comprehensive and up-to-date database of community services (e.g., housing, food assistance, healthcare, legal aid, educational programs).Guide individuals and families through the process of accessing services, providing direct referrals and follow-up to ensure successful connection and engagement.Create and conduct needs assessment surveys (community, program and individual)Deliver area specific small-group workshops on navigating healthcare, social service systems, and civic resources.Design and facilitate community-based educational sessions, workshops, and presentations on topics such as financial literacy, health education, tenant rights, and digital literacy.Collaborate with subject matter experts and community partners to tailor curriculum to diverse audience needs and advance CityTeam’s mission to enable lasting solutions.Promote self-advocacy by equipping community members with knowledge of their rights and the skills to navigate systems independently building them up to support one another in the group, their homes and community.Work closely with Lasting Solutions staff members in other locations to create surveys, share resources, work cohesively to create curriculum that will serve multiple locations and create pathways for the Elevate II programming pipelines and case management.Conduct outreach to underserved populations, build relationships with community members, and serve as a trusted point of contact for information and anize and participate in community events, fairs, and public forums to raise awareness of available services and educational opportunities while elevating CityTeam’s brand and presence in the Mayfair Community.Partner with local service providers, educators, and community leaders to coordinate efforts, exchange information, and develop integrated solutions.Maintain accurate and timely records of client interactions, referrals, educational participation, and outcomes.Assist in tracking performance metrics and preparing reports for program evaluation and dividualized case management and life skills navigation.PLEASE NOTE: To perform this job successfully, an individual must be able to perform each of the essential duties satisfactorily.
