Learn more about UHS at uhs.berkeley.edu.ResponsibilitiesAssists and supports Eating Disorders team by providing case management of patients, coordinating care between multiple UHS services (e.g., Social Services, Nutrition, Primary Care) and/or external providersProvides case management for Eating Disorders patients in collaboration with Primary Care, Urgent Care, Social Services counselors, and Registered DietitiansCase management may include regular patient visits, responding to patient-initiated contacts, identifying patient-centered solutions to barriers, facilitating referrals, health system navigation, documentation, advocacy, and coordination of multidisciplinary case conferencesDevelops a patient-centered care plan with psychosocial and psychological techniques (e.g., behavioral activation, problem solving, goal setting, motivational interviewing, harm reduction)Documents patient progress and treatment recommendations in the EHR and other systems to facilitate information sharingTracks patients who have not followed-up and coordinates with ED Treatment Team to address barriers to adherenceParticipates in weekly Eating Disorders Team MeetingsAssists with medication management as prescribed by Primary Care and medical providersParticipates in caseload consultations and addresses barriers to treatment adherence for patients not currently participating in treatmentProvides support for patients awaiting initial Eating Disorders counseling appointments and communicates acute issues to providersFollows up with at-risk patients who miss initial intake appointmentsCollaborationWorks with all clinic providers, support staff, and supervisors to ensure integrated health servicesSupports colleagues by offering assistance and sharing informationParticipates in regular and ad hoc consultation meetings to discuss patient care and outcomesEnsures appropriate access to care for patients referred to the Eating Disorders serviceParticipates in a care management teamCommunicationInvests in organizational and unit goals to improve patient serviceDevelops collaborative relationships and communicates through appropriate channelsObtains and shares relevant information to enhance teamwork and patient educationParticipates in staff meetings and demonstrates empathy and professionalismAssesses learning needs and educates patients and staffHandles conflicts professionally and adheres to University and UHS policiesUses shared decision-making with patients and communicates plans to the care teamAssists with monitoring and tracking patient navigation across UHS and off-campus referralsConveys information from patient contacts to care providers via the EHRCare Management DevelopmentIdentifies system and practice improvements and advocates for leadershipCollaborates to develop protocols improving care for at-risk patients and supports ED providersMaintains and shares internal resources to promote efficient care, including coordinating with community providers for off-campus referralsHelps develop a care management team for student mental health and coordinates services across UHSOrganizational ResponsibilitiesAdheres to quality, customer-oriented standardsMaintains professionalism and confidentiality in patient and staff interactionsParticipates in committees and program development when assignedDocuments inquiries and complaints appropriatelyUnderstands patient rights and confidentiality rulesAssumes additional responsibilities and ensures completeness of patient recordsKnowledgeable of UHS and campus services and policiesCollaborates with UHS staff and adapts to evolving healthcare practicesRequired QualificationsEducationMasters in Social WorkMasters in CounselingLicenses & CertificationsCurrent California state license (LCSW or MFT) or ability to obtain by start dateKnowledge, Skills & AbilitiesAdvanced case management and psychosocial/psychotherapeutic skillsExperience with high-risk populations and eating disorders in a college settingStrong collaboration with diverse patient groups and health staffEffective verbal/presentation skills and computer literacyKnowledge of applicable laws and professional conductProficiency in coordinating care and communicating in a multidisciplinary environmentPreferred QualificationsAt least 3 years post-licensureExperience with multicultural and marginalized college students in university counselingWorking knowledge of electronic health recordsInterest in continuing development of this roleSalary & BenefitsFor information on UC Berkeley’s comprehensive benefits package, visit the Compensation & Benefits site. The posted salary range is Step 1 $89,616.56 – Step 11 $109,263.07. This is an 80% full-time (32 hours/week) exempt position eligible for UC benefits.
