OverviewRequisition No:861313Agency: Department of HealthWorking Title: HUMAN SERVICES ANALYST - 64006332Pay Plan: Career ServicePosition Number:64006332Salary: 36,000.00 AnnuallyPosting Closing Date: 09/28/2025Total Compensation Estimator ToolJob posting category: Open CompetitiveSpecial Note: Multiple positions may be filled.Your Specific ResponsibilitiesProvides medical case management to persons infected and affected by HIV/AIDS within the four-county area of Indian River, Martin, Okeechobee, and St. Lucie Counties (area 15).Delivers proactive and inclusive case management activities through office or field visits according to the Florida Department of Health Bureau of Communicable Diseases HIV/AIDS Section and the Area 15 HIV/AIDS Program Office guidelines; reports to the Operations Management Consultant pletes comprehensive Needs Assessments of individuals seeking assistance.Performs face-to-face assessments and re-assessments (including assessment of treatment adherence, appointment adherence, and family planning needs); documents medical, physical, and psychosocial conditions and identifies service needs and barriers; assesses client resources and strengths; ensures medication and appointment adherence issues are addressed; completes acuity assessment at initial assessment; intake/enrollment screening for initiation of case management.Develops individualized care plans addressing identified needs; integrates assessment findings into the Plan of Care; conducts ongoing monitoring and periodic re-evaluations (at least every six months) and updates acuity as needed; ensures continuity of care and closes gaps in service.Implements and oversees care plans; determines type and frequency of contact based on client acuity; conducts follow-up to ensure service delivery.Ensures payer source hierarchy is used appropriately; screens for Medicaid, PAC Waiver, Medicare, VA benefits, and private insurance; utilizes Ryan White as payer of last resort; updates payer information in CAREWare and HMS databases within three business pletes and maintains accurate documentation of all client-specific activities, including direct client interactions, case notes, form scans, and database entries within required timeframes.Provides counseling, support, education, and treatment adherence monitoring; educates clients about HIV, transmission, and risk reduction; discusses insurance and entitlements and provides navigation and enrollment support; advocates for clients with providers and other services as needed.Coordinates linkage to agency, community, and government services; facilitates re-engagement; monitors referral completion for programs such as ADAP and PAP; coordinates transportation as needed; collaborates with Ryan White, HOPWA, prevention, and other partners to ensure continuity of care.Participates in meetings and trainings; completes required trainings and timekeeping; manages administrative duties including required documentation and reporting.Conducts Acuity Assessments covering basic needs, co-morbidities, dental, housing, mental health, risk behaviors, transportation, insurance, and treatment adherence; adjusts contact frequency based on acuity level.Required Knowledge, Skills, and AbilitiesKnowledge of interviewing techniques, effective communication, and dealing with diverse populations; ability to maintain client confidentiality.Proficiency in English language, spelling, grammar, and administrative writing; strong attention to detail and independent judgment under stress.Ability to work independently or with minimal supervision; strong planning, organization, and workload prioritization skills.QualificationsMinimumOne year experience at a social service-related agency or other social services position.Emergency duties may require work before, during, or beyond normal hours, including work in special needs or Red Cross shelters.PreferredBilingual English/Spanish or English/Creole; or tri-lingual (English/Creole/Spanish).Florida Department of Health Mission, Vision, and ValuesMission: To protect, promote & improve the health of all people in Florida through integrated state, county & community efforts.Vision: To be the Healthiest State in the Nation.Values: Innovation, Collaboration, Accountability, Responsiveness, Excellence.Where You Will WorkFort Pierce/Vero BeachThe Benefits of Working for the State of FloridaBenefits include:Annual and Sick Leave benefitsNine paid holidays and one Personal Holiday each yearState Group Insurance coverage optionsRetirement plan options with employer contributionsAdditional information at and RequirementsThis position requires a security background check and/or drug screening and participation in direct deposit.
