Your Specific Responsibilities This position provides medical case management to persons infected and affected by HIV/AIDS within the four-county area of Indian River, Martin, Okeechobee, and St. Lucie Counties (area 15). Provides proactive and inclusive case management activities through office or field visits according to guidelines established by the Florida Department of Health Bureau of Communicable Diseases HIV/AIDS Section and the Area 15 HIV/AIDS Program Office. Reports to the Operations Management Consultant II. Completes comprehensive Needs Assessments of individuals presenting themselves for assistance through the agency, including: Face-to-face assessment and re-assessment (adherence to treatment, adherence to appointments, family planning needs). Documentation of client medical, physical, psychosocial condition and needs; identifies service needs addressed for the client and barriers to care. Evaluates client resources and strengths, including social support, for care planning; ensures adherence issues are addressed. Completes acuity assessment at initial intake and within 30 days of the brief; conducts intake/enrollment screening to initiate case management. Develops an individualized care plan addressing identified needs; ensures findings flow into the Plan of Care and case notes; conducts re-evaluation at least every six months and updates acuity as needed; monitors continuity of care and gaps in care. Care Plan Implementation, Client Contact, Monitoring, and Follow Up—oversees care plan implementation, completes tasks, and conducts contact based on client acuity; follows up to determine service delivery. payer sources—Ensure hierarchy of payer sources is used appropriately; screen for Medicaid, PAC Waiver, Medicare A/B/D, VA benefits; determine private insurance status; refer to ADAP Premium Plus when appropriate; ensure Ryan White services are used as payer of last resort; update payer source eligibility in CAREWare and HMS databases within three business days. Documentation—complete and maintain accurate client-specific activity records, including direct interactions, case notes, form scans, and information in CAREWare and HMS databases within three business days; document inactive/closed cases and refer to community resources; follow up with providers as applicable. Counseling, Support, Education—monitor HIV medication therapy, educate on risks/side effects, support treatment adherence, review CD4 and viral load values, educate about HIV, transmission, risk reduction, insurance/navigation, and client advocacy; coordinate linkages to agency/community/government services and monitor referral completion (e.g., ADAP, PAP, SSI, SSDI, Medicaid, Medicare, housing, etc.). Coordinate and link clients to additional services (housing, substance abuse, mental health, ADAP, food programs, etc.); assist with transportation for medical and social services; collaborate with other departments to ensure smooth service transitions and continuity of care. Administrative—participate in conferences and meetings; complete mandatory trainings/webinars; submit required timesheets and documentation in required timeframes. Acuity Assessments—conduct assessments across basic needs, comorbidities, dental, family planning, health insurance, housing, mental health, risk behaviors, self-sufficiency, substance use, transportation, and treatment adherence; re-evaluate acuity according to level (Low/Moderate/High) with specified contact frequency and case conferencing as warranted. Required Knowledge, Skills, and Abilities Knowledge of techniques for effectively dealing with people; interviewing techniques; effective communication. Knowledge of English language structure, spelling, grammar. Ability to be detail-oriented, think independently, exercise good judgment, work under stress, and handle periodic workload peaks. Ability to work with a diverse population and maintain client confidentiality. Ability to prepare correspondence and administrative reports; work independently or with minimal supervision; plan, organize, and coordinate work; communicate effectively orally and in writing; prioritize workload. Qualifications Minimum One year of experience at a social service-related agency or other social services position. Incumbent may be required to work before, during, and/or beyond normal work hours in case of emergency; emergency duties may include work in special needs or Red Cross shelters or other disaster responses.
