JOB SUMMARY Main purpose: to deliver of hot meals to eligible home-bound seniors/elders, and on site service to other tribal and community members. To transport seniors/elders to and from medical, dental and other appointments. MAJOR TASKS AND RESPONSIBILITIES Transports seniors/elders to medical and/or dental appointments and shopping, etc. Assists patients in way-finding to their medical/dental appointments. Assists in medication drop-offs for seniors/elders unable to get to pharmacy. Picks up medications for seniors/elders at tribal/local pharmacies. Responds to inquiries by phone and in person, identifies client's needs, and assists in scheduling appointments as appropriate. Operates tribal and/or GSA vehicles, including handicap van. Assists patients in and out of vehicle when appropriate. Assists with operation of wheelchairs and other durable medical equipment. Keeps schedule book; is prompt to appointments. Allocates travel time appropriately. Data entry of transportation and other services rendered - TAMS system. May be required to transport hot meals to home-bound seniors/elders. Is familiar with local providers' office locations, hospitals and medical/dental facilities. Because of the Tribe's commitment to community service and the well being of its members, each employee may be expected to perform a wide range of office and field duties from time to time. Such duties may or may not be related to their regular responsibilities. EDUCATION - EXPERIENCE AND TRAINING FOR POSITION High school diploma or equivalent. Current and valid Washington State Driver's License. SPECIFIC SKILLS/KNOWLEDGE/ABILITIES REQUIRED FOR POSITION Basic first aid CPR Office equipment operation, including phones, computers, copiers, faxes, pagers Confidentiality/HIPAA policies Verbal and written communication Some medical knowledge and skills Knowledge and recognition of prescription drugs Good communication skills with medical personnel and peers Ability to make quick and sound decisions in case of emergency Recognizing diabetic shock and other emergency medical situations Interpersonal relations Multi-tasking, organization Working as part of a multi-disciplinary team Meeting deadlines, being prompt and on time Providing fair and equitable customer service PHYSICAL REQUIREMENTS The working conditions described herein are representative of those that must be met by an employee to successfully perform the essential duties of this class. The employee is frequently required to sit and hear, push/pull, use feet to move controls on vehicles, machinery or equipment, and to drive a motor vehicle. The employee is occasionally required to talk, stand, walk, climb stairs, bend while assisting clients in and out of vehicles, reach with hands and arms, grasp with the fingers, and use hands to finger, handle, or feel objects, tools or controls. The employee must occasionally lift and/or move up to 40 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. While performing the duties of this job, the employee occasionally works in outside weather conditions. The employee may occasionally be exposed to exhaust fumes, biohazards, and/or communicable diseases. May also occasionally be exposed to excessive noise, vibration, electric shock, and moving mechanical parts. The noise level in the work environment is usually moderate. Licenses or Certifications Required A valid Washington State driver's license is required at the time of appointment or at a time set by the Tribe.
