Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency.We currently have an opportunity for a Field Nurse Supervisor in New Jersey.The role of the Field Nurse Supervisor is to provide professional nursing services in compliance with governmental regulations, individual State Nursing Practice Acts and Premier policies, procedures and philosophies.Qualifications:Educational: Minimum of R.N. Diploma from an NLN approved Nursing ProgramTravel: Travel may be required for business purposes. If so, the employee must have a valid drivers license issued by the state in which they work and a satisfactory driving record.Work Experience: Minimum of one year of experience in medical surgical nursing preferably hospital based preferred. One additional year of home-health based experience is required for conducting applicable training classes.Bilingual (Spanish and Asian) preferred.Must maintain all state and federal compliance requirements for licensure and employment.Additional Requirements:a) Knowledge of regulatory requirements to Joint Commission/CAHC/DOHb) Fundamental knowledge of the basic principles of nursing and the ability to apply these principles in a home care settingc) Ability to accurately assess patient health care status and psycho social needsd) Ability to establish a Plan of Care which will assist the patient to reach desirable health statuse) Ability to function with minimal supervisionf) Reliable means of transportation.Essential Job Responsibilities:Institute contact with physicians as necessary.Confer with patients physician in ascertaining the need for home care and in developing, implementing and revising the Plan of Care.Perform initial patient assessment on those patients admitted to the agency for care.Develop an individualized goal outcome Plan of Care.Provide and supervise care according to the Plan of Care.Ensure that physicians orders are obtained initially, reviewed as required and communicated appropriately.Assist with home maker and HHA supervision.Develop the Aide Plan of Care reflecting initial patient assessment and Plan of Care.Assess the appropriateness/effectiveness of all paraprofessional caregivers on a regular basis and annually.Submit and maintain accurate and timely documentation to client care.Document supervisory visits.Other related duties as assigned.EOE/M/F/D/VJob Type: Full-timeBenefits:401(k)Dental insuranceHealth insuranceHealth savings accountLife insurancePaid time offVision insuranceLanguage:Spanish (Required)English (Required)License/Certification:RN License (Required)Work Location: On the road
