Core ValuesDoing What it Takes, Honoring Choices, Its Personal, Teach, Engage, Act, Make it Meaningful.Job SummaryThis is a full-time professional position responsible for responsible for oversight and management of the intake and admission services for the organization, statewide. This position will oversee the day-to-day clinical operations, budget management, daily admissions, and performance outcomes of the intake, admissions, referral center and program representative staff. This position supervises the admission nurses, clinical liaisons, and the Intake and Referral Manager. ResponsibilitiesResponsible for the administration of all intake and admission programs and services statewide. Implement and administer clinical referral inquiry to admission operations, policies, and procedures.Assures customer service excellence.Monitors and ensures staff compliance with established hospice referrals and admissions processes and procedures through ongoing oversight, coaching, and performance evaluations. Conducts regular tracers and audits to assess adherence to protocols, identify areas for improvement, and provide targeted support and training to maintain high standards of quality, efficiency, and regulatory compliance.Provide leadership, support and direction to the Admission and Referral Team to ensure timely response to referral inquiries, admission assessments and effective communication to internal and external customers.Works cooperatively with the Hospice Services team and maintains compliance with Medicare/Medicaid, State and Federal Regulations, NHPCO standards and JCAHO standards.Plans and maintains adequate staffing including evenings and weekends. Assure staff productivity and recommend changes in staffing standards as appropriate. Responsible for the interviewing and hiring of staff within the admissions department and referral center. Identifies staff development needs in conjunction with Human Resources, assures appropriate staff development through in-service programs. Encourages continuing education, certification, and coordinates attendance at outside in-services as able.Develops, updates, and implements policies and procedures with input from direct reporting staff. Utilizes the company provided systems for all work and analyzes data daily for decision making. Assures the ongoing provision of admission and intake staff including mandatory education requirement and the development of education program in response to staff needs.Proactively works to deploy program representatives and admission staff to meet the needs of referral sources and patients. Establishes and maintains effective relations with community and professional groups and the staff of agencies with which the organization has contractual agreements. Assures ongoing in-services and relationship management with referral sources, providers, and facilities.Collaborate with all departments in the organization for effective growth.Identifies opportunities for performance improvement. Continually evaluates and acts on opportunities for improvement within the referral and admissions department. Participate in appropriate meetings to provide effective communication and problem-solving regarding management and staff goals within the organization.Explores innovations within the Hospice Healthcare industry and evaluates opportunities for inclusion in company services. Makes recommendations for opportunities, strategies, and improvements to process.Participates in budget development. Assures admission operations within the existing budget. Adheres to companys Ethical Practices.Requirements Bachelors degree in nursing or necessary clinical experience required.Nursing License or equivalent in Delaware and Pennsylvania required. Masters degree in nursing or management/administration preferred.Minimum of 5 years of clinical experience with at least 1 year in hospice. Minimum of 2 years of experience in management of people. Excellent communication and people skills. Strong leadership abilities.Ability to work as part of a team to reach shared goals. Highly organized, detail oriented, and accurate.Excellent computer skills preferred.Must be able to travel to enter and function in the workplace.May be required to lift and/or carry items up to 30 pounds.May be required to work for extended hours in front of a video display terminal and use repetitive motions. Must have sensory abilities to complete work assignments, communicate with internal and external customers and to complete the reporting and follow-up documentation responsibilities of the job.
