Scheduled Hours40Position SummaryProvides individual attention to each patient and needed support by navigating patients through the complexities of the screening, diagnostic, treatment, and maintenance care continuums.Job DescriptionPrimary Duties & Responsibilities: Serves as a resource to assist patients and family navigate care continuum, barriers to care, and identifies resources/interventions related to social determinants of health (SDOH).Tracks interventions and outcomes.Completes and processes all necessary paperwork and documentation to enroll patients in appropriate programs.Facilitates communication and education with heath care staff and providers.Performs other duties as assigned.Working ConditionsJob Location/Working ConditionsNormal office environment.Direct patient care setting.Physical EffortTypically sitting at desk or table.Repetitive wrist, hand or finger movement.Ability to move to on and off-campus locations.Occasional lifting (EquipmentOffice equipment.The above statements are intended to describe the general nature and level of work performed by people assigned to this classification.
