**Responsibilities** **As a Nurse Practitioner** with CommonSpirit Health at Home, you'll do more than assess symptoms - you'll bring comfort, clarity, and compassionate care to patients during some of the most meaningful times of their lives. In collaboration with our Medical Director and interdisciplinary care team, you'll play a key role in evaluating ongoing eligibility for home health and hospice care.
