Job DetailsJob LocationCommunity Based - Wheeling, WVDescriptionJob Summary: The Family Coordinator provides meaningful partnerships with families and youth with the goal of improving functioning in their homes, schools, and communities. This employee contributes to the prevention of opioid-related overdose, increase access to treatment, reduce stigma and support families affected by opioid crisis. The Family Coordinator promotes coordination and integration of family-centered care, facilitate participation and involvement of the entire family toward treatment and recovery and connect families with supportive services. This employee will ensure compliance with safe work practices and maintain safe environments in compliance with established agency standards, licensing regulations and generally accepted practices. *Employees required to use their personal vehicles in certain circumstancesDuties and Responsibilities: Increase positive outcomes for youth receiving treatment and recovery servicesConduct active client outreach services with high risk population focus. Increase engagement of family members and caregivers for youth and young adults receiving treatment and recovery services.Assist clients and families with system level navigation to resources for themselves and families to facilitate participation in servicesMaintain schedules for each family referral to ensure completion of treatment servicesIncrease retention of family member and caregiver participation in treatment and recovery programs.Develop and implement a plan to track, disseminate and educate clients on the use and limitations of naloxone and fentanyl stripsMaintain active record of all services providedResponsible for assuring proper discharge from servicesIncrease number of referrals of families and youth/young adults to community supports, resources to improve overall wellbeingIncrease communication among stakeholders and awareness of collaboration opportunitiesProvides community programming and outreach across Youth Services' five-county region: Hancock, Brooke, Ohio, Marshall, and Wetzel Counties. Facilitate the formation and continue a regional family advisory committee.Partner with other child-serving agencies to increase cross-referral and utilization of existing resources with the state. Complete required grant reporting and other selected data-sharing processes. Complete all necessary documentation of services, monthly reports and discharge summaries.Attend all required meetings and training. Complies with federal, state, and company policies, procedures, and regulations. Demonstrate knowledge of and support Youth Services mission, vision, and values, policies and procedures, operating instructions, confidentiality standards, and code of ethical behavior. Must have a valid driver's license and proof of automobile insurance as required by law
