Opportunity Overview: * Working in the town of Las Vegas, NV * 15 -20 patients per day * Part-time position * Nurse practitioners (NP) and physician assistants (PA) are welcome to apply * Position: Advanced Practice Clinician (APC) * Compensation and Benefits: Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated income range of $50,000 to $75,000 annually with no cap on productivity bonus income potential. Responsibilities: * Drive high-quality, safe, and cost-effective patient care in post-acute care communities * Inpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospital * Collaborate with other clinician interdisciplinary teams and follow CMS guidelines * Work closely with hospice, palliative care, and home care agencies for safe patient transitions * Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines Qualifications: * Board certified FNP, AGACNP, ACNP, AGPCNP, or PA-C * Willing to train experienced candidates as well * Licensed in state California Applicant Privacy Act:
