Schedule: Full Time 8am-430pm Monday- Friday Shift: Days Location: 1 Salt Creek Lane, Hinsdale, IL UChicago Medicine Advent Health Cancer Institute Hinsdale * Please Apply if you have Oncology Experience. * The role you'll contribute: Serves as a patient navigator throughout the continuum of care. The value you'll bring to the team: + Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers. + Coordinates efforts for treatment and promotes quality improvement and educational efforts. + Compiles data, tracks outcomes, and makes recommendations for process improvement. + Provides referrals and supports for follow-up care for patients who are uninsured or underinsured. + Develops and maintains relationships with physicians, post-acute providers, therapists, patients, and families. + Collaborates with care management, physicians, and clinicians to develops discharge plans requiring post-acute services. + Ensures seamless transitions across the continuum of care. + Educates patients and families about post-acute care services and expectations. + Assists in assessing patients for post-acute care and coordinates the clinical transition to home health, hospice, or other appropriate settings. + Maintains comprehensive knowledge of managed care and community resources. + Completes and submits all documentation in a timely manner according to department policy. + Participates in multidisciplinary rounds, care conferences, and coordination with case management. + Other duties as assigned.
