Overview Ui Health - Clinical Nursing Consultant II - Transplant Hospital (Liver) Location: Chicago, IL USA • Requisition ID: 1036726 • FTE: 1 • Work Schedule: Days, M-F Position Summary The Clinical Nursing Consultant II (Transplant Liver Coordinator) develops and implements nursing educational programs for nursing staff within a clinical service. The CNC II facilitates patient care delivery using the Nursing Process and Relationship-Based Care. It provides patient care and consultation, coordinates staff education, and acts as a clinical role model. The role aims to improve patient outcomes by integrating Shared Leadership, Evidence-Based Practice, Relationship-Based Care, and nursing excellence into practice. Through therapeutic relationships with patients, families, self, and colleagues, the CNC II participates in monitoring, developing, and evaluating nursing practice while supporting unit and organizational goals. Responsibilities Evaluate patients at appropriate UI Health sites of care. Create and maintain transparent patient care workflows to ensure safety and support team dynamics. Provide emotional support to patients and families throughout the transplant process. Coordinate care for transplant recipients from referral through waitlist, including living donor and related pathways. Ensure patient coordination is documented and complies with UNOS, CMS transplant program guidelines, and UI Health policies. Collaborate with providers to order evaluations as needed and review results in a timely manner per UNOS and CMS guidelines. Develop, coordinate, and communicate the patient’s plan of care based on ongoing assessment and family input. Utilize clinical expertise to collaborate with multidisciplinary teams to improve outcomes. Consult with and implement provider orders, documenting in the electronic medical record. Gather clinical information from the team, patient, and family to develop an educational plan. Recognize symptoms, establish priorities, and respond to changing patient needs in the transplant continuum. Ensure the patient is added to UNOS waiting lists and that listing letters are sent promptly per UNOS requirements. Coordinate and schedule living donor transplant surgeries as assigned. Attend transplant QAPI, listing, department, and multidisciplinary meetings as needed. Provide cross-coverage for the transplant data coordinator by entering living donor information in UNOS as assigned. Engage nursing and support personnel by delegating tasks per the Illinois Nurse Practice Act and provide feedback. Demonstrate commitment to UI Health values, teamwork, and effective communication to achieve quality outcomes, safety, satisfaction, and a safe environment. Integrate theoretical knowledge, evidence-based research, current standards, and experience to provide high-quality care. Participate in Shared Leadership and governance, volunteering for at least one area of shared governance. Collaborate with nurses and physicians to promote learning experiences for staff and students. Initiate and participate in evidence-based practice research to improve patient care outcomes. Serve as preceptor, mentor, and resource for nursing staff and students. Encourage obtaining board certification related to transplant (e.g., ABTC or CCTC) within two years of hire. Attend practice-related educational opportunities (conferences/workshops). Identify and communicate learning needs to the manager; receive feedback constructively; participate in educational/outreach activities. Other duties as assigned. Minimum Qualifications High school diploma or equivalent. Graduation from an accredited school of nursing. Eligibility for registration with the Illinois Department of Financial and Professional Regulation. Two years of transplant nursing experience (RN). Current AHA BLS/CPR and other required unit-specific certifications. Bachelor’s in Nursing (BSN) required. Effective oral and written communication skills for therapeutic relationships with patients, families, and providers. Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR). Current Illinois nursing license required. Preferred Qualifications Transplant or organ-specific certification preferred (must obtain within 2 years of employment). MSN strongly preferred.
