Job DescriptionSpecialty accepted: FNP, ANP, GNP, PA Start date: ASAP - ongoing Shifts requested: Monday Friday 8am 6pm Practice setting: Home Health Patient load: 5 to 8 assessments per day Ages: Geriatrics Procedures required: H&P evaluations; physical exams; medication reviews; vitals (BP, urine, finger stick); provide fecal kit for lab submission Requirements: Active and unrestricted TN NP or PA license, CAQH, NPI, willingness to travel at least 75 miles, completion of online training modules, Medicare/Medicaid eligible (not opted out).
