Direct Support Professional I Job Description This role involves providing care and support services in a Home and Community-Based Services (HCBS) Setting, offering medical, social, and supportive services to individuals with diverse needs. As a key member of the care team, you will empower individuals to achieve their goals and reach their full potential across various aspects of life. Collaborate with the HCBS Supervisor and Programmer to develop personalized goals for the individual served. Deliver services tailored to each individual's unique needs and behavioral care plan. Key Responsibilities: Responsibilities include: Teaching individuals skills to accomplish their goals rather than completing tasks for them. Implementing behavioral strategies that support individuals and address behaviors hindering their success. Motivating and encouraging individuals to participate in their behavioral care plans and work towards their objectives. Participating in recreational, community, and social activities with those you serve, respecting the choices of the individual. Displaying an understanding of when it is appropriate to apply rights-restrictions, as determined by the interdisciplinary team and indicated in the individual's behavioral care plan. Additional Responsibilities: Include: Providing accurate and consistent documentation of the individual's progress. Ensuring safe environments and instructing individuals on practices to help them reach their goals. Providing care that enables individuals to present themselves confidently in their community. Advocating for individuals receiving services to maintain control over their lives.
