HCBS Care Specialist

This role operates in a HCBS Setting, defined as medical, social, and supportive services for individuals with functional, cognitive, and other physical or mental health needs. Rather than institutionalizing those who have these needs, HCBS settings provide the opportunity to live and receive services in a way that honors their abilities. Direct Support Professionals are essential in delivering high-quality care to individuals. What Success Looks Like: In this role, you'll be responsible for developing personalized plans to support individuals in achieving their goals. This includes: Collaborating with supervisors and programmers to develop goal-based plans Providing services based on unique goals and behavioral care plans Key Responsibilities Include: o Teaching individuals how to accomplish their goals rather than completing tasks for them o Applying behavioral strategies to support individuals and address any behaviors that don't lead to their success o Motivating and encouraging individuals to participate in their behavioral care plans and reach their goals o Participating in recreational, community, and social activities with those you serve while honoring their choices o Displaying an understanding of when it's appropriate to apply rights-restrictions, only as determined by the interdisciplinary team and indicated in the individual's behavioral care plan Providing complete, consistent, and accurate documentation of the individual's progress Creating safe environments and instructing individuals on practices that will help them achieve their goals Providing care to individuals that helps them present themselves positively to the community Advocating for individuals to have control over their lives and be treated with respect Benefits and Opportunities: We offer a range of benefits to support your well-being and career growth.
