This position provides proactive and inclusive case management activities through office or field visits according to the guidelines established by the Florida Department of Health Bureau of Communicable Diseases HIV/AIDS Section and the Area 15 HIV/AIDS Program Office. This position reports to the Operations Management Consultant pletes comprehensive Needs Assessments of individuals presenting themselves for assistance through the agency. Includes face-to-face assessment and re-assessment (adherence to treatment, adherence to appointments, and family planning needs). The assessment describes the client’s medical, physical, and psychosocial condition and needs, identifies service needs addressed for the client, barriers to care, and services not adequately coordinated. It also evaluates resources and strengths, including social support networks, for care planning. Needs Assessments indicate that medication and appointment adherence issues have been addressed. Completes acuity assessment at the time of the initial comprehensive needs assessment. The comprehensive needs assessment is completed within 30 days of intake. Intake/enrollment screening initiates case management activities and collects pertinent information.Develops an individualized care plan to address identified needs. Address individualized goals, objectives, needs, and barriers to care. Findings of the Needs Assessment must flow into the Plan of Care and be addressed in the case notes. Documentation must show ongoing addressing of needs. Re-evaluation at least every six months with adaptations as necessary according to acuity level over the life of the client. Completes acuity assessment at the time of the care plan update. Monitors clients to assess implementation and efficacy of the care plan, ensuring continuity of care and no gaps in care.Care Plan Implementation, Client Contact, Monitoring, and Follow UpResponsible for the implementation and oversight of the care plan. Completion of tasks listed in the care plan. The type and frequency of contact should be based on client acuity. Case managers follow up to determine delivery of services.Ensures that any applicable hierarchy of payer sources is used appropriately. Screens for Medicaid eligibility, PAC Waiver eligibility, Medicare A, B, and D eligibility. Verifies VA benefits. Determines whether the client has private health insurance or could obtain private health insurance through an employer, referring to ADAP Premium Plus when appropriate. Ensures Ryan White services are consistently used as the payer of last resort. Updates all required information regarding payer source eligibility on clients in the CAREWare and HMS Databases within three business pletes and maintains accurate documentation of all client-specific activities.Documentation includes direct client interactions and roles undertaken on behalf of a client. Includes written case notes and form scanning. Detailed case notes describe how client needs are being addressed, barriers to meeting needs, and progress toward Plan of Care goals. Case notes, form scans, and all required information on clients are entered in the CAREWare and HMS databases within three business days. Documentation on inactive and closed cases is completed, exploring all contact methods and referred resources, and follow-up with providers of record.Counseling, Support, EducationMonitor HIV medication therapy, provide education on risks and side effects, monitor adherence and tolerance of medications, and provide treatment adherence support. Review and monitor CD4 and viral load values, ensuring most current values are recorded in the client file/database. Educate clients about HIV, transmission, complications, risk reduction, insurance, entitlements, and provide navigation and enrollment support. Client-specific advocacy with landlords, medical teams, substance use counselors, etc. Facilitate linkage to agency and community services and monitor referral completion (including ADAP, PAP, SSI/SSDI, Medicaid, Medicare, housing programs). Coordinate, refer, and link clients with additional services such as housing support, substance abuse and mental health treatment, ADAP, and food services. Authorize transportation for medical and social services as needed. Network with other departments to ensure smooth transition of services and continuity of care.AdministrativeParticipate in conference calls, staff and planning meetings regarding Ryan White Case Management duties, DOH and other Ryan White patient care meetings. Complete required trainings/webinars. Complete EARS/CSRS and timesheets within required timeframes, including related documentation submission.Acidity and Required Knowledge, Skills, and AbilitiesCompletes Acuity Assessment covering basic needs, co-morbidities, dental, family planning, health insurance, HIV state, housing, mental health, risk behaviors, self-sufficiency, substance use, transportation, and treatment adherence. Client contact frequency is based on acuity level with minimums defined. Case conferencing with treatment team as warranted by acuity level.Required Knowledge, Skills, and AbilitiesKnowledge of techniques for dealing with people, interviewing, and effective communication. Knowledge of English language structure and grammar. Must be detail-oriented, able to think independently with good judgment, perform under stress, and handle fluctuating workload. Ability to work with a diverse population and maintain client confidentiality. Ability to prepare correspondence and administrative reports.
