Job Details * 15 patients per day * Full-time position: 5 days per week, Monday through Friday * Compensation and benefits: our compensation is fee for service (FFS); expected compensation is estimated salary range of $132,184 to $156,250 annually with no cap on productivity income potential - this full-time role is benefit eligible Key Responsibilities * Perform primary care in a post-acute setting for short term and long-term patients * Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care * Order and interpret lab results * Prescribe and monitor medications * Consult with physicians and coordinate patient care with other healthcare practitioners * Educate patients and their caregivers on coping skills to handle chronic and age-related conditions * Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery * Promote healthy lifestyle changes Qualifications * A minimum of 1 year experience as a nurse practitioner or physician assistant in a primary care setting required * Post-acute care experience a plus, but not required * Active and unrestricted New York NP or PA license required * Active and unrestricted DEA with prescriptive authority required Respond with your CV to this opening to learn more about joining our team!
