You’ll collaborate with physicians, social workers, and other healthcare professionals to ensure patients receive efficient, cost-effective, and compassionate care.This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needsBenefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insuranceA 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility datesRN CASE MANAGER – HOME HEALTH | RESPONSIBILITIES:Perform comprehensive assessments and create individualized care plans.Continuously reassess patient needs to ensure appropriate levels of care.Coordinate care transitions and collaborate with multidisciplinary teams.Implement non-medical support such as housing or transportation when needed.Monitor and document clinical, functional, and fiscal outcomes.Identify opportunities for prevention and health promotion.Advocate for patients, ensuring privacy and confidentiality at all times.RN CASE MANAGER – HOME HEALTH | REQUIREMENTS:Education: Associate’s Degree in Nursing (BSN preferred).Attendance: Commitment to mandatory attendance during the first 90 days.Current unrestricted RN license (multi-state license required).Case Management certification.OASIS and Homecare Homebase (HCHB) experience required.CPR/BLS certification.Ability to analyze complex medical information and make sound clinical decisions.Take the Next Step!Join a company that values your growth and contributions.
