Social Worker

{EMPLOYER-DESC} is seeking a compassionate and dedicated Social Worker to join our team. In this role, you will have the opportunity to make a significant impact on the lives of individuals and families by providing support, guidance, and resources to help them navigate through challenging times. You will work closely with healthcare professionals to ensure a holistic approach to patient care. Assess client needs and situations to develop effective service plans. Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life. Participate in the development of treatment plans with other healthcare professionals to ensure comprehensive care. Provide crisis intervention as necessary. Refer clients to community resources for services such as food stamps, childcare, healthcare, and housing. Prepare and maintain reports and case files, including documentation such as clients' treatment plans, progress notes, and correspondence. Advocate for clients and patients? rights and services.
