Family Nurse Practitioner Clinic Hours: M-F 8:30am – 7:30pm, Sat 9:00am – 5:30pm, Sun 9:00am – 4:30pm  Employment Type: Full time 32-40 hours a week 00 per hour / $156,000 per year Family Nurse Practitioners provide evidence-based, primary and family care in collaboration with a dedicated clinical care team. You will manage a panel of patients and help drive better outcomes through assessment, diagnosis, treatment, chronic disease management, preventive care, and health counseling. Deliver primary care services: assessment, diagnosis, treatment, health counseling, and disposition planning Provide patient education on healthy lifestyle choices and preventive care Coordinate care with other healthcare professionals Open and close the clinic, and maintain clinic inventory Master’s degree in Family Nurse Practitioner program State license to practice as an Advanced Practice Nurse DEA Registration required at time of offer Fully vaccinated against COVID-19 (prior to start date, unless exempt) At least 1 year of recent Primary Care, Urgent Care, Family Medicine, Retail Clinic experience as an FNP or PA in a family medicine or primary care setting Experience managing chronic conditions and understanding quality measures Bilingual skills a plus Technologically proficient (EMR use, telehealth collaboration) Comfortable practicing independently and remotely within a clinical team model You’ll be part of a care team that supports your success through daily collaboration, training, and shared goals.
