Position Summary: We are seeking an individual who is passionate about rehabilitation, eager to grow clinically, and thrives in a team-oriented environment. The ideal candidate will provide efficient, high-quality care that focuses on improving patient’s mobility and function. The therapist will work closely with an interdisciplinary team to support patient recovery, mobility, and the discharge plan. Key Responsibilities: Perform evaluations of patients physical and functional status Develop appropriate goals based on clinical findings Educate patients and families to support active participation in care Collaborate with physicians, nurses, care managers, occupational therapists, speech-language pathologists, etc. to provide coordinated, holistic care. Maintain timely and accurate documentation in accordance with departmental and regulatory standards. Contribute to a positive team environment that values respect, empathy, accountability, and innovation Position Requirements : Graduate of approved School of Physical Therapy. Licensed as a Physical Therapist in the State of Illinois. Certified in CPR through the American Heart Association.
