Annual Salary: $30/hr $40/hr (varies depending on years of experience)We are seeking a compassionate and motivated LMSW to join our clients multidisciplinary care team at a community-based health center in Brooklyn, NY. This role focuses on providing culturally sensitive psychosocial support to individuals and families, with an emphasis on underserved populations. The ideal candidate will be fluent in Spanish and passionate about improving access to behavioral health services in a collaborative, patient-centered environment. Key Responsibilities:Conduct psychosocial assessments and develop individualized care plans in coordination with medical and behavioral health providers. Provide short-term counseling, crisis intervention, and supportive therapy to patients dealing with mental health, substance use, or social challenges. Assist patients in navigating community resources, including housing, food assistance, legal aid, and other social services. Advocate for patients needs within the healthcare system and with external agencies. Maintain accurate and timely documentation in the electronic health record system. Participate in case conferences, team huddles, and interdisciplinary planning meetings. Support care coordination efforts, including follow-up on referrals and transitions of care. Educate patients and families about mental health conditions, treatment options, and coping strategies. Contribute to quality improvement initiatives and program development as needed. Requirements:Masters degree in Social Work (MSW) from an accredited institution. Active LMSW license in the state of practice. Minimum of 12 years of experience in a healthcare, community, or social service setting. Fluency in Spanish is strongly preferred to support our diverse patient population. Strong interpersonal and communication skills, with the ability to build trust and rapport across cultures. Familiarity with trauma-informed care, motivational interviewing, and integrated care models is a plus. Proficiency in electronic health records and documentation standards.
