The Primary Care Physician (PCP) in our organization demonstrates accountability for outcomes, acts as a health coach, and simplifies care to drive action. Overview ChenMed is a physician-led and mission-driven primary care provider. We seek PCPs who are focused on purpose and personal opportunity and who can help us achieve our mission. The PCP is responsible for delivering high-quality, cost-effective care for a panel of up to 450 patients and will lead a care team to support patient health and center performance goals. Essential Job Duties/Responsibilities The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient. The PCP takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed, while remaining involved in the detailed care of the patient. Engages with hospitalists for hospitalized patients (regardless of whether the hospitalist is part of ChenMed). Assesses, diagnoses, treats, manages, educates, promotes health, coordinates care, and documents for patients with acute and complex chronic needs. Leads the care team (care promoter, care facilitator, and care coordinator) for patients who can come to the office. For patients unable to come to the office—hospital, SNF, LTC or homebound—engages with transitional care and other resources including case managers and related physicians depending on the market. Manages center operations, covers for other providers as needed, and participates in recruiting patients and providers as required. Supervises and collaborates with an Advanced Practice Practitioner (APP) within a practice or collaborative agreement and remains accountable for the APP’s actions. Performs other duties as assigned and modified at manager’s discretion. Evidence-Based Medicine — stay updated on clinical, nutritional, and lifestyle interventions and recognize where factors outside traditional medicine impact health outcomes. Physician Leadership — commit to developing leadership skills for the benefit of patients, the team, the center, and the company. Quality — strive for high-quality outcomes and continuously improve practice and center operations. Influence — effectively influence patients and teams to achieve the best outcomes. Self-Care — maintain physician wellness for sustainability and the health of patients and staff. Proficient in Microsoft Office Suite and related software used in the company. Ability and willingness to travel locally as needed, and nationally for initial training and occasional recruiting or training purposes. Fluency in English (verbal and written); some centers may require other language fluency as noted at the time of application. Independent judgment in professional practice. Education and Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required. Must be able to obtain or currently hold an active State Medical License for the state(s) of work. Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required. Once board certified, maintain board certification through required MOC, CME, and/or retaking exams as required. Current DEA number for schedule II-V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment.
