This leadership role works in the context of the Service Line & administrative leadership, and as such is integral to its success. Candidate will be required to assess needs of the program and implement improvements necessary for expansion of the program.Expectations:Partner with the Department/Division: Meets with the chair/division chief to inform, update, solicit feedback on key initiatives, engage and obtain support from departmental/divisional leadership on initiatives.Role Model: Model high quality performance demonstrated through attention to professional behavior, communication, responsiveness to patients and staff, flexibility, consistent practice with quality and safety standards, and support of our culture of safety and patient-centered care.Physician Champion: Be the physician champion and clinical leadership partner for access, quality, cost, and optimization efforts for advanced lung disease and transportation care in a manner that actively engages providers, care team and the practice patient population. These include performance improvement activities, Joint Commission ongoing readiness, patient experience improvement, care delivery redesign, etc.Partner with Interdisciplinary Team: Join with dyad partners for Thoracic Transplant and Pulmonary and Critical Care Division along with interdisciplinary teams to model collaboration around achieving strategic goals in both specifically designed mechanisms of alignment (e.g. Service Line P&P team, and informal interactions to support the formation of high performing local teams.)Education: Partner with the Departments/Divisions and F&MCW to optimize the clinical education experience for learners including, but not limited to: fellows, residents, medical students, nurses, technicians, and visiting physicians.Research: Partner with the Departments/Divisions and F&MCW to participate in relevant clinical trials and translational research.Specific duties:Creating the care philosophy & programmatic policies guiding the care of advanced lung disease patients throughout the continuum including device therapies & transplantation (in collaboration with Thoracic Surgery and Pulmonary & Critical Care Division leadership)Implementing the Lung Transplant outreach business planImplementing strategic initiatives to include navigation and structured handoffs for patients, care coordination, & support for patient’s self-management goals, risk stratification, advanced care planning and best practice care for pre-lung transplant patients in both inpatient & outpatient care settingsProviding consistent physician leadership & engagement at selection committee and Lung P&P team (75% attendance required)Understanding & following regulatory requirements, and serving as a partner for survey preparedness & compliance initiatives.
