Adult Primary Care Physician

OverviewWe are seeking Primary Care Physicians who are certified in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics. Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants. The program focuses on providing comprehensive primary care and social services to keep seniors living in their homes and communities. You will work within an interdisciplinary team (IDT) and report to the Chief Medical Officer to deliver patient-centered care.ResponsibilitiesProvide primary care and acute illness evaluation and treatment for a panel of approximately 150 participantsPerform comprehensive assessments for participants and coordinate with the IDT to develop a comprehensive care planBe an engaged member of the IDT, communicating participant changes and coordinating care planning and 24-hour care deliveryCollaborate with network providers and other IDT members to ensure effective care coordination around hospital discharge and placement decisionsProvide direction and guidance to Nurse PractitionersOversee participants/' use of medical specialists and inpatient careParticipate in rotating night and weekend callOversee all care of participants in the nursing home with regular visits as dictated by PACE standardsReview outcome and utilization data to identify opportunities for improved care and operationsQualificationsSuccessful completion of a residency program accredited by the Accreditation Council for Graduate Medical EducationMaster's degree or doctorate degreeBoard Certification in Internal Medicine or Family PracticeBoard Certification in Geriatrics preferredActive California Physician License without restrictionsState CDS Certification and DEA RegistrationValid state driver/'s license with a clean DMV recordCopy of recent vehicle insurance requiredExcellent clinical, organizational, and communication skills in settings with frail, elderly participants and the IDTCurrent knowledge of chronic care/geriatric issues and best practicesBase and BenefitsBase pay range: $250,000.00/yr - $280,000.00/yrCompensation: $250,000-$280,000 with quarterly bonusMedical insurance coverage (Medical, Dental, Vision)Work/life balance with 17 days of personal time off, 12 holidays, and sick time401(k) savings + matchAdvancement opportunities and potential to become a medical directorCME and Paid Time AwayMalpractice InsuranceRelocation assistance up to $50,000 and a $10,000 sign-on bonusRotating on-call schedule, generally a few nights or weekends per monthJob DetailsSeniority level: Mid-Senior levelEmployment type: Full-timeJob function: Health Care ProviderIndustries: IT Services and IT Consulting #J-18808-Ljbffr
