Primary Care Physician

The Primary Care Physician (PCP) leads a patient panel of up to 450 and collaborates with care teams to achieve high-quality, cost-effective outcomes. Essential Duties and Responsibilities Independently provide care for patients with acute and chronic illnesses in the older adult population. Take full accountability for patient care and outcomes; seek appropriate specialist consultation when needed while remaining involved in the detailed care of the patient. Engage with hospitalists for patients admitted to the hospital, regardless of hospitalist affiliation. Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document for patients with acute and complex chronic needs. Lead the care team (care promoter, care facilitator, care coordinator) for office-based patients; coordinate with transitional care teams for patients unable to visit the office (in hospital, SNF, LTC, or homebound). Active role in center management, coverage for colleagues as needed, and participation in recruiting patients and providers as applicable. Performs other duties as assigned by management. Knowledge, Skills & Abilities Availability and accessibility to build trust with patients, including reasonable walk-in access and timely responses to calls/messages. Service orientation: provide care as you would want for a family member. Evidence-based medicine: stay up-to-date on clinical, nutritional, and lifestyle interventions that impact outcomes. Physician leadership: ongoing development of leadership skills to benefit patients, teams, centers, and the company. Quality: strive for high-quality outcomes and continuous improvement of practice and center operations. Influence: competently and compassionately influence patients and teams to achieve the best outcomes. Self-care: maintain physician wellness to sustain patient, staff, and community health. Proficiency with Microsoft Office and related software; adaptable to various digital tools used by the company. Ability and willingness to travel locally as needed; possible national travel for initial training and occasional recruiting or training. Fluency in English (verbal and written); other languages may be required for certain centers. Ability to exercise independent judgment. Education and Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related specialty required. Active or eligible State Medical License for the state(s) of practice. Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred; Board eligibility required. Maintains board certification through MOC, CME, and/or retaking exams as required after board certification. Current DEA number for schedule II-V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within 90 days of employment.
