LVN -Outpatient- Level II - Rancho Cucamonga MOB - Family Medicine-Regular Clinic - Part TimeJob SummaryProvides nursing care to patients under the direction of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to observe nursing intervention and assists medical staff to ensure optimal care within established policies, procedures, and standards. Under the direct supervision of a Registered Nurse or provider, provides nursing care in accordance with the legal scope of practice and within established standards of care, policies, and procedures. Demonstrates performance consistent with the Mission, Vision, and Strategic Plan of the organization. Treats coworkers, patients, families, and all members of the health care team with dignity and respect. Essential Responsibilities Exhibits excellent customer relations with patients, visitors, providers, and co-workers; shows courtesy, compassion, and respect; conforms to the department’s critical service standards. Provides therapeutic patient care in accordance with the physician/advanced practice provider and/or a registered nurse within the scope of practice. Transcribes physician/advanced practice provider orders and acts under policy. Communicates with a registered nurse to institute changes in the plan of care; assists with and contributes to the development of a nursing care plan. Administers specified medications appropriately and accurately following the provider’s order and/or standing orders within the scope of practice. Executes treatments such as urinary catheterizations, placement of intravenous catheters, bladder irrigations and instillations, suctioning procedures, throat irrigations and enemas, and uses equipment as required; adheres to infection prevention and control policies and procedures. Maintains infection prevention practices and adheres to disinfection policies and procedures. Performs related patient care functions (e.g., taking vital signs, wound care, patient education, specimen collection) and assures correct culture collection. Assists physician/advanced practice providers or other medical staff with examinations and treatments, providing instruments, support, and patient handling as required. Observes and documents patient conditions; identifies significant findings, initiates emergency procedures as necessary, and reports to the RN/APP per policy. May volunteer to start and monitor IV therapy; may discontinue IV therapy as appropriate; may perform venipuncture/blood withdrawal with certifications. Collects and processes specimens per policy and procedures. Provides non-complicated teaching following the plan of care; identifies learning needs and discharge barriers. Ensures cleanliness and safety of the nursing station, patient exam rooms, and equipment. Maintains effective verbal and non-verbal communication. Performs duties in other departments or modules as required to maintain standards of care and safety. Supports back-office workflows including care gap closure, patient outreach, and nurse visits. Participates in staff meetings and in-service programs; assists in ordering and stocking supplies and equipment. Accepts verbal and telephone orders from the physician/advanced practice providers. Supports patient communication and message management (emails, center messages, back-office messages). Administers respiratory support as ordered, including pulse oximetry, humidification, spirometry, oxygen therapy, suctioning, and sputum induction as appropriate. Documents care in Health Connect per policy, procedures, guidelines, and protocols. Performs other related duties as assigned, including orientation of new employees and coordinating discharge processes. Performs all job functions within license certification and scope of practice; promotes customer service and upholds the organization’s standards and code of conduct. Experience Minimum twelve (12) months LVN direct patient care experience within the last three (3) years (e.g., medical office, acute care, home health, skilled nursing facility).
