What You’ll Be Doing Lead care across 8–12 post-acute and long-term care facilities (volume and locations vary). With a thoughtful approach to rounding and a trusted clinical team by your side, you’ll bring clarity, comfort, and consistency to the patients who need it most. Design and manage your own weekday rounding schedule Provide skilled and custodial visits aligned with regulatory standards Collaborate with Nurse Practitioners and care teams Support patient and family communication with heart and clarity Document visits with clarity using SOAP format in EMR Schedule & Lifestyle Flexible M–F rounding, you’re in control No shift work or clinic hours Daytime phone coverage only (7am–7pm) Optional weekend or overnight call available for additional compensation After-hours coverage provided so you can fully unplug Who You Are (a.k.a.
