POSITION DESCRIPTION Job Title & Code: Advanced Practice Professional (APP) - HVI - Cardiothoracic Surgery - Thoracic Job Codes: Nurse Practitioner (85002), Physician Assistant (85102) Department: Heart and Vascular Institute Reports To: Direct Supervisor/Director of Advanced Practice Professional of the Heart and Vascular Institute FLSA Status: Exempt POSITION SUMMARY The APP coordinates and manages the care of the CT surgery patient across the continuum of care in collaboration with CT Surgery providers. The APP oversees the involvement of medical/surgical specialty teams to develop a comprehensive plan of care, directs that care with the clinical team, and ensures adherence to protocols and regulatory guidelines. The APP orders/performs diagnostic/therapeutic procedures, interprets studies, coordinates peri-operative services, and discharge/follow-up planning. The APP participates in education of residents and medical/APP/nursing students. Provides healthcare services consistent with state law and licensing/certification requirements and hospital privileges, as applicable. MINIMUM QUALIFICATIONS For Nurse Practitioner Education, Certification, and/or Licensure: Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing National Certification by one of the recognized APRN Certification Centers Licensure, or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia BLS certification required. ACLS at department discretion APRNs seeking First Assist privileges in peri-operative services must complete CNOR via ANOR-approved program and pass CNOR exam Experience: Eligible new graduates will be considered Preferred Qualifications CT Surgery, Acute care or cardiology experience preferred For Physician Assistant Education, Certification, and/or Licensure: Bachelor’s or Master’s Degree as a Medical/Surgical PA (as applicable) Graduate of an ARC-PA approved PA program Certification by the National Commission for Certification of Physician Assistants Licensed or eligible for licensure by the WV Board of Medicine BLS certification required; ACLS at department discretion Experience: Eligible new graduates will be considered Preferred Qualifications Masters level education as Medical/Surgical PA (as applicable) preferred CT Surgery, Acute care or cardiology experience preferred CORE DUTIES AND RESPONSIBILITIES The following describe the general nature of work performed by people assigned to this position. They are not all-inclusive. Other duties may be assigned. Manage comprehensive and individualized care for CT surgery patients and families in inpatient and post-discharge settings. Perform daily rounds, pre-procedure evaluations, and consults during weekdays and as applicable on weekends/holidays. Complete integrated assessments and document clinical evaluations/plans in the EMR. Order and interpret labs and diagnostic studies; coordinate peri-operative services and referrals as appropriate. Communicate diagnostic results and coordinate care with the patient’s care team, following clinical guidelines and scope of practice. Develop a comprehensive plan of care, including teaching, discharge planning, and referrals as needed. Identify unstable or complex patient care situations and coordinate interventions with appropriate resources. Respond to emergencies and initiate appropriate emergency care when needed. Facilitate discharge by coordinating services and tests for outpatient follow-up as applicable. Provide care across the age continuum and maintain surgical skills for first assist in CT surgery operations and conduit harvesting where applicable. Assist in data collection for outcome measures and participate in interdisciplinary approaches to patient care, program development, and education. Educate nursing staff, hospital staff, students, patients, families, and the community; promote ongoing learning and competency development.
