Job Summary Hospice LVN provides nursing care to terminally ill Hospice patients as needed. As an LVN, you will evaluate the patient’s overall health. Your goal will be to improve the quality of life for your patients. All nursing will be done in accordance with each patient’s individual plan of care. Responsibilities Provide services within the scope of Nursing, in accordance with the plan of care, and in coordination with the interdisciplinary Team Follow plan of care; provide treatment; evaluate patient’s reaction to treatment Report any changes in his/her condition to the hospice Clinical Director Online Charting Kinnser in a timely manner on company-provided devices Be responsible for following all company policies and procedures Perform other duties as designed Requirements One year of previous nursing experience One year of Hospice or Home Health experience (preferred but not required) Current LVN License to practice in California Current CA Driver’s License. #J-18808-Ljbffr
