This role is a safety-sensitive position as it involves direct patient care and administering medication. The incumbent will report to a Regional Manager (or similar role) and will work closely with other healthcare professionals to provide quality care to patients collaborating with other clinical teams to support team function.ResponsibilitiesPractice medicine using sound medical judgment and adhere to all ethical considerations of the practice of medicine; know limitations and seek consultation when advisable; keep abreast of current technologies and industry standardsMeet all clinical obligations of the medical practice: Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, dependently provide care for patients with acute and chronic illness encountered in the older adult patientAccountable for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will stay involved in, and be responsible for, the detailed care of the patientEngage with hospitalists whenever a patient is in the hospitalAssess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelinesMaintain complete and detailed records, electronically in the EMR, of patients’ health care plans and prognosis, completed within 48 hours of each patient’s visitProvide quality evidence-based medical care in a managed care environment, focused on prevention, and outcomesExhibit behavior that emphasizes compassion, quality, loyalty, and conscientiousnessInterpret data and diagnostic results for an appropriate course of action including but not limited to blood gasses, radiology, EKG, etc. Ability to establish and maintain effective working relationships with patients, their families, and team membersOversee and supervise Nurse Practitioners and/or Physician Assistants when requestedResponsible for ensuring that patients are seen at least every other visit by a PhysicianProvide 5-Star service in accordance with Better Health Group's missionAdditional duties as assignedPosition Requirements/ SkillsMedical Degree from an accredited medical schoolActive medical license in the state of practiceAt least 3 years of experience in outpatient primary care experienceACLS/BLS certification is required as mandated by state of practiceExcellent diagnostic skills and ability to interpret medical tests and resultsPrior experience working in an electronic medical records systemAbility to diagnose and manage chronic diseases such as diabetes, atherosclerotic disease, COPD, CHF, obesity, etc. in the senior populationAbility to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulationsAppreciation of cultural diversity and sensitivity toward target patient populationsWorking knowledge with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaborationExcellent organizational and time-management skills with strong attention to detailAbility to shift focus, multitask, and prioritize in a rapidly changing environmentDemonstrated ability to handle emergencies calmly and effectivelyDemonstrated resourcefulness, initiative, and results-oriented capabilitiesMust be able to work professionally with confidential informationStrong reasoning, critical thinking, and problem-solving skillsMust be results-oriented with a focus on quality execution and deliveryAbility to work cross-functionally with multiple teams; ability to work independently with minimal supervisionExcellent written and verbal communication skills; must be comfortable communicating with senior-level leadership, providers, and health plansStrong interpersonal skillsDemonstrated ability to handle data with confidentialityPhysical RequirementsFull range of body motion, including handling and transferring patients, manual and finger dexterity;
