Centers offer a team-based care model where physicians lead a multidisciplinary team to support physical, emotional, and social wellness. The Primary Care Physician (PCP) works as a lead in a team-based care environment focused on quality of care for the patients we serve.Our care team includes Doctors, Advanced Practice providers, Pharm D, Care Coach Nurses, Medical Assistants, Behavioral Health, Specialists, Quality Based Coders, Referral Coordinators and more. Our model emphasizes higher quality care and better outcomes for seniors through a concierge experience, multidisciplinary services, coordinated care supported by analytics, and deep community relationships.ResponsibilitiesEvaluates and treats center patients in accordance with standards of care. Follows level of medical care and quality for patients and monitors care using available data and chart reviews.Assists in the coordination of patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment and home health care. Participates in daily huddles as a clinical expert and supports a cooperative practice tone with Regional Medical Director and Center pletes medical record documentation in a timely manner with a quality-based coder to optimize coding specificity. Follows policy and protocol defined by Clinical Leadership. Meets with RMD about quality of care, review of outcome data, policy, procedure and records issues.Participates in growth opportunities for new or existing services within the Center. Participates in the local primary care on-call program as needed. Ensures personal compliance with licensing, certification, and accrediting bodies.Spends 100% of time clinically focused on direct patient care, including patient-facing time and related administrative tasks (charting, meetings, etc.).QualificationsCurrent and unrestricted medical license or willingness to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment as required.Graduate of an accredited MD or DO program.Excellent verbal and written communication skills; strong interpersonal relationships with colleagues and patients.Fully engaged in an Integrated team-based care model; willingness to learn/adapt to a value-based care setting.Superior patient/customer service; basic computer skills including email and EMR.TB screening program participation may be required if selected.Active DEA license; Medicare Provider Number; Medicaid Provider Number.Preferred qualifications: Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.Two to five years of directly applicable experience preferred; experience managing Medicare Advantage panels and knowledge of Medicare guidelines.Bilingual is a plus; knowledge of HEDIS quality indicators is helpful.Benefits and Additional InformationGuaranteed base salary + quarterly bonusExcellent benefits package effective on first day of employmentCME Allowance/Time; Occurrence-based Malpractice InsuranceRelocation and sign-on bonus options; 401(k) with employer matchLife Insurance/Disability; Paid Time Off/HolidaysMinimal on-call requirementsBe careful - Don’t provide bank or credit card details when applying for jobs.
