Location:Remote - must live in or near Ashe, Alleghany, or Stokes County, North Carolina.General Statement of JobThe Family Partner is or has been a primary caregiver (e.g., parent; legal guardian) of a child with serious emotional, intellectual/developmental, behavioral, substance use, or mental health concerns. They have knowledge of the behavioral health system and provide coaching, modeling, and education of system protocols and processes. The Family Partner supports children, youth, and families, adhering to System of Care (SOC) values, and collaborates with multiple service providers to offer individualized, trauma-informed advocacy. They work with community stakeholders, support development of local Community Collaboratives, and provide training, support, and leadership opportunities to families and agencies. The role covers a three-county area in collaboration with the SOC Coordinator.Essential Job FunctionsFamily Support/Child and Family TeamsThe Family Partner actively participates in Child and Family Teams (CFTs) to ensure:Children, youth, and families are prepared for meetings and understand the process.Family members are encouraged to contribute to agendas and invite lists.Children, youth, and families actively participate, identifying strengths, needs, and goals.Plans align with the family's culture, language, and values.Services are strength-based, flexible, and available in the community.CFTs include natural supports and community munication uses child- and family-friendly language.Services involve evidence-based practices and informal supports.Concerns are addressed, and services are adjusted as needed.The Family Partner provides ongoing support, including advocacy, resource linkage, and collaboration with CFT members and other staff. They also facilitate community connections and leadership opportunities for munity Engagement and OutreachThey ensure diverse community representation in Collaboratives, actively recruit members, and participate in community events to promote SOC and behavioral health services.Training and Capacity BuildingThe Family Partner assists with or delivers training to stakeholders and families on SOC principles, family-driven practices, and strength-based processes.Evaluation and Quality ImprovementThey lead or support efforts to evaluate SOC effectiveness, advocate for high-quality care, maintain documentation, and participate in related meetings and trainings.Collaboration with State and Community BodiesThey represent their counties in statewide meetings, participate in subcommittees, and meet all Department standards, including attending webinars, meetings, and site visits.Professional DevelopmentThe Family Partner receives SOC training, participates in professional development, and attends relevant meetings and trainings.Knowledge, Skills & AbilitiesThorough knowledge of SOC values and principlesEffective collaboration with diverse stakeholdersUnderstanding of family dynamics and needsKnowledge of Person-Centered Planning and service protocolsAbility to document in electronic records while maintaining confidentialityValid NC driver’s licenseQualifications & EducationHigh school diploma or GED required, with at least four years of experience as a primary caregiver for a child or youth receiving Medicaid or state-funded behavioral health services. Within 12 months of employment, completion of SOC training from NC DMH/DD/SAS is required.Mental/Physical RequirementsMust have close visual acuity, ability to perform physical activities such as walking and reaching, and lift up to 10 pounds. Sedentary work required, with extended sitting periods.Residency RequirementMust reside in North Carolina or within 40 miles of the state border.SalaryDependent on qualifications and experience.
