CSO is looking for a Family Partner for our Community Behavioral Health Center (CBHC) in Athol.Are you the parent or caregiver of a child who has struggled with emotional or physical challenges? Was there ever a time that you felt like you were alone? There are many other caregivers out there navigating their way through a similar journey, and they could use your support. CSO has a unique opportunity for you to share your lived experience and provide guidance and insight to those going through challenges similar to the ones you've already endured.This is a peer position, for a candidate who has lived experience as a caregiver of a child who's received services for mental health diagnosis.You WillAs a Family Partner:Work in conjunction with CBHC Clinician and other staff to complete assessments and Family Support Plans (including CANS Assessment when needed)Provide support for parent and/or family members through process of assessment and Family Support Plan development; encourage involvement of natural supports for familiesOversee implementation of the Family Support PlanProvide direct supports to family membersDeliver services on-site or in the parent/caregiver's home or communityWhen applicable, provide education, coaching, training and assistance to parents in navigating child serving systems (DCF, DMH, juvenile justice)For families engaging in Intensive Care Coordination services, participate in Wraparound processes including completing paperwork, attending Care Plan team meetings, and communicating with Care CoordinatorAs a designated critical employee of CSO, must come to work when scheduled regardless of adverse weather conditions, and when other CSO offices are closed, including holidays. In the event there are extenuating circumstances, employee must make prior arrangements with Program Director in order to maintain adequate staffing in the programComplete all initial and ongoing credentialing requirements for those hospitals where employee will be providing services including obtaining required vaccinations, tox screens, and safety/health education competencies.
