Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of adult home health experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Reliable transportation - Current driver’s license - Agency-required liability insurance - Recent home health experience Description: Home visits will be conducted. Periodic office meetings are required. The average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. Each type of visit and activity (documentation, travel, etc.) holds a different point value. Contractor must be willing to travel up to 75 miles from home base. Mileage will be reimbursed at the IRS standard rate. Commuting to and from the first and last patient is not billable. Must be comfortable with daily driving to patient homes and the clinic. All documentation must be completed within 24 hours of the patient appointment. Delayed and incomplete documentation that impacts billing and follow-up care may result in cancelation. There are no guaranteed hours with this contract. Clinician must meet weekly productivity expectations. Must accept every patient on the scheduled visits regardless of distance from the previous appointment. No weekend rotation is required. Traveling to nearby locations may be necessary to maintain patient care hours. Home health experience is essential for this role.
