Description: Candidates must review the credentialing checklist prior to submission. State license verification is required. Resume and three references are needed. County criminal background check for seven years for all counties lived and worked is required. Federal criminal background check for seven years or more is mandatory. Multi-state criminal search covering seven years or more is required. Nationwide sex offender search covering seven years or more is necessary. HHS/OIG search must be performed. GSA search must be performed. Social security number trace is necessary. A two-step PPD or Quantiferon or proof of a positive PPD and chest X-ray must be provided. SAM search is required. Oregon state background check must be completed.
