About the Opportunity Medix is working with a premier client to fill a vital Travel Physical Therapist opening at their facility in Kokomo, IN. This assignment offers a unique and rewarding experience working within a Program of All-Inclusive Care for the Elderly ( PACE ), a holistic model dedicated to helping seniors live safely and independently in their homes and communities. You will be joining a brand new PACE center, which provides a stimulating, "YMCA-style" environment for active and engaged elderly participants. This is an exciting chance to help shape the center's patient load as the census grows, with the possibility of extensions and future assignments at new centers opening across the country. Location: Kokomo, IN Facility Type: Outpatient at a PACE facility Start Date: Early November Length of Assignment: 13 Weeks (Option for 6 months) State License Needed: Valid State Licensure as a Registered Physical Therapist (PT/DPT) Shift: Monday - Friday, 8:00 AM - 4:00/4:30 PM (5 eight-hour shifts). Call/Weekends: No on-call, weekends, or evening shifts. Role & Responsibilities As a Physical Therapist, you will plan and administer physical therapy treatment programs for PACE participants to maintain and/or restore function, relieve pain, and prevent further disability. Your day-to-day duties will include: Assessment & Treatment: Completing evaluations, providing treatments, and ramping up to a caseload of approximately 10 scheduled patients per day as the center census increases. Therapy Focus: Majority of therapy treatments will focus on generalized strengthening, balance, fall prevention, and helping participants increase mobility and overall physical activity. Coordination: Coordinating care with the participant's Primary Care Physician (PCP) and participating in developing interdisciplinary care plans and care team meetings. Equipment Management: Assessing the need for and ordering, fitting, and educating participants on adaptive equipment and durable medical equipment (DME) such as walkers, wheelchairs, braces, etc. Documentation: Completing all required documentation, including assessments, progress notes, and discharge summaries, utilizing the IntusCare (PACE-specific) EHR system. Supervision & Instruction: Developing, instructing, and supervising other staff or family members on therapy programs. More About the Setting Patient Population: Elderly participants (age 55) who are active and mobile enough to participate in the PACE program. Facility Amenities: You will be working in a new center equipped with a full, brand-new gym, including parallel bars, bands, weights, and balls.
